Allow WBL eligibility form fields to be used to create an experience

When choosing a form as a WBL eligibility form, create field mapping rules to align each form field (by name in the template) to the data needed to build what is required to create a new experience.

• Employer record (company name at minimum)

• Employer Contact record (first name, last name, email at minimum)

• WBL Experience record (WBL type at minimum, set start date to today)

Allow all other relevant options to be mapped as well.

Add an option to the eligibility datatable to Create Experience. We would then attempt to determine if the employer/contact are new or existing.

Give the user a confirmation screen to allow them to override the new/existing employer/contact information
