When adding a new on-site visit and uploading a file attachment is is not saved and the attachment count is shown as 0

This only seems to be an issue when initially creating a new on-site visit entry. If you edit an existing one and upload an attachment, there are no issues.

    We were not able to replicate the issue of the files not saving but did resolve the attachment count problem.
