Unclear Prerequisite for Mileage Reimbursement Page

When trying to add a Mileage Reimbursement Signature Page and leaving the On-Site Visit Form Template option blank, a non-descript error is shown. We should make the error clearer and/or hide the mileage reimbursement page option until an on-site visit template is selected if that is indented to be a prerequisite.
