Training Plan PDF Attachment Not Displaying After Upload

Form has an attachment field built into the template. When you submit the form-WBL coordinator is first signer. Able to select a file but the system doesn’t show it attaches. The field remains as Choose File. However, you can submit the form after selecting your file and the file is attaching-you just can’t see it until after you sign/submit it.
