Student Applications - Allow applicant name (form initiator) to be a prefill option on form templates

Currently, if an applicant submits a teacher recommendation form where they are intentionally not a signer, there is no way to prefill their name on the form. This is essential because the recipient of the recommendation form has no way to know who they are submitting a recommendation for. The student can’t be a signer because the teacher’s recommendation must be confidential and not shared with the student.

A universal solution would be to add prefill options for the Form Initiator (first name, last name, full name, email address).
