Student Applications - Add an option designate that an application template is for Adult Ed

If this is set, the Counselor Information could be repurposed to allow the adult applicant to provide more information after the application is accepted. This could also make the Parent/Guardian consent form into an approval form.

    The counselor information and approval has stayed the same for now, but other changes have been deployed.

    The main thing that is needed here is to remove parent/guardian information requirement and grade level (or set it to 13).

    Checkbox should remove unnecessary columns and change the name of the form to approval form.
