Reports - Create a Weekly WBL Hours report

There should be a row for each student and columns for a weekly date range with 2 sub-columns (total hours approved & total hours pending).

Before generating the report ask the user for the start and end dates (default to all), WBL Types (default to all), and add a final Grand Total Hours column. There should be standard fields for the student such as first name, last name, email, grade level, grad year, dob, program, school, sending school, state student number, and local student number as well.
