For tracking progress, we need to know the following. 

• School

• Student Last Name

• Student First Name

• Student number

• grade

• class name

• class code

• class section number

• teacher

• experience type

• status of training agreement (who is the next signer, completed, etc)

• status of other enrollment forms

• has the experience been marked as completed / archived?

• Start Date

• End Date
