Pathful Forms not allowing submission

Pathful forms are not allowing students to submit the form. After hitting “submit” the page spins but nothing actually gets submitted. In addition, if students exits screen to try again, all documents uploaded are gone and they have to start over with the upload, only to have the same thing happen again.
