OAC Minutes: Checkbox in Section 6C "Denied" unchecks on submission

User reports a bug in OAC minutes submission: a checkbox in section 6C (labeled "Denied" by user, possibly "Motion Not Approved" in Action Taken section) automatically unchecks itself upon submission, causing the submission to be rejected.
