K12-360: There are scrolling issues on Learner Progress Notes.

The trash can (delete) icon is absolute positioned and it should be relative to the message.  This causes alignment issues when you scroll.  Also, the delete icon should appear for newly created messages.  Finally, the chat icon should show for any outcome that has notes applied to it (on refresh, this appears to disappear).
