Industry Certifications - Add ability to track attempts (pass/fail) and record notes (similar to task progress)

When running reports only the most recent pass/fail status and date need to be listed but failures need to be included if the test was not yet passed.

Notes could be used to document what score they received and to show the history of previous attempts (the way it is done for task progress).
