Forms - When sending back for revision, allow multiple fields to be selected

Currently, you can only select one field, but multiple may need to be addressed. It is also not really evident that after selecting just one field that the form will be sent back. It would also be helpful to list a reason for each field separately so the original signer knows what needs to be addressed for every field.
