Forms / Notifications - allow notifications to be sent to certain users based upon form field data

For instance, if a form was sent out to all WBL students asking them if an employment change occurred and they chose Yes, a notification could be sent to a WBL coordinator to alert them of this change. For the majority that chose No, no notification would be needed.
