Forms - If you edit an existing form template, it should only add new fillable fields but it is adding duplicates

For instance, here you can see where the template already had fields defined and yet uploading the same PDF file caused the fields to be duplicated.

    I believe this is resolved but it’s not fully tested. If this comes back up, please re-open with example forms.
