Forms - Add a field type of Date/Time with formatting option to choose the Date/Time format to insert

For fields like employment start/end date and begin/end time, having a form field type that would display a Date/Time selector would be beneficial. It could be a single field type option with settings for Date, Time, or possibly Both and a formatting option that would use standard PHP DateTime formatting options.

    Instead, I opted to make "Date" and "Time" data type options on text fields. This will automatically format these to "mm/dd/yyyy" and "hh:mm am/pm". When the signer is filling it out, they will have a date/time picker where specified.

    We have date fields on forms like this:

    The "Expected H.S. Graduation Date" is currently defaulted to the "Student Graduation Year"

    Is it also possible to default it to a date MM/DD/YYYY?

    Possibly pop up a calendar to select from?
