This is the only time I’ve ever seen this. I also looked in the school’s instance and there are other students who have filled this out and the values are in the correct places. If I were to bet, the parent has access to the students email and didn’t realize they were filling it out on their behalf or did know and mixed them up. That’s the only theory that I can come up with for this. If this happens again, we will investigate further.
