Feature Request: Add 'Type of Representative' options for MI State Compliance

For the State Advisory Template for CIP Self Review, I need to list the Advisory Representation. Since I am unable to change the report I want to add more options tot he drop down for Type of Representative. I will attach the form that I need to have filled out to be compliant. This form list all of the options that I need to be able to select.
