Date, Integer, Decimal, and Calculation not functioning in Form Signer

In the Updated -Weekly Hours Log form the form fields are configured with decimal, date, and a calculation. These fields are not acting correctly in the form signer after the form is submitted. Dates are not consistent, hours entered do not stay, and the calculation is not "mathing".
