Career Planning - Add an option for an activity to be assigned to a Teacher

A specific example of why this may be needed: In NY, schools need to track employability profile forms for every student. This should be part of the Career Plan, but the Teacher is the one that needs to initiate the form. The teacher is the only signer and it has progressions (Y1 S1, Y1 S2, Y2 S1, Y2 S2) so the dates need to be set.
