Allow users to log in to SignMyForm to see a copy of any form that they have ever signed.

This would give us the option to allow users to opt-out of receiving notifications about forms being completed and give them an alternative means to receive a copy of completed forms to ensure we are still complying with the ESIGN Act requirements.
