Allow student-specific forms to be completed for WBL events

For example, if a school has a Media Waiver form that students need to sign before participating in an event, there needs to be someway to submit and view whether that is complete. Another example would be for schools that do SBE or SL as events and need to have a training plan and/or training agreement signed for each student.
