Allow schools to have dynamic areas of a form somehow or allow submitted data from distinct forms to be consolidated for reporting

This came up as an issue for statewide systems. For example, there is one Training Plan that is expected to be used for all school across the state. However, each school would like their contact information, logo, etc. to appear on the form. This is also an issue for form templates that are exported from one application and imported into another.
