Allow a coordinator to enter data for all fields of a form and then upload a scanned copy so all the data from the form is captured and reportable

Since the idea is to have all data in digital form for state reporting, this will allow that data to be captured. We would show the uploaded form when viewing since it will contain the signatures, then add the data entered for each field into the database. This would potentially also need to be done for forms where a form is partially completed digitally and then a completed form is uploaded later. In that instance, the coordinator should only need to enter the fields that were blank.
