Advisory Boards - Allow signature pages to be optional

Some people may not want to use digital forms.  They may just want to upload admin response for instance. It would be nice to have a checkbox to disable each.

If disabled for admin response, disable submit button and only allow uploads. For meeting minutes, just generate a pdf from the data entered instead of submitting a form (and still allow uploads).
