Add state-specific codes and point values to industry certifications

Currently we have the PA-approved industry certifications with the state code assigned to each. Other states have similar lists, such as Ohio.

Additionally, Ohio assigns a point value to each certification to identify the difficulty level of achieving each certification. To obtain a graduation requirement, students must achieve 12 points.
