Add State Approved / Local and Code column to Proposed Industry Certifications table

Currently it just shows the Student Name, Certification, Attachment, Date Achieved and the Approve/Deny.

We should update it so that the Industry Certification column shows the Organization - Name with the State Approved or Local badge. We should add a column for Code after the Industry Certification column.
