Add "Required by All Students" checkbox to Student Document Types

Currently, if a Student document type is created, it’s automatically required by and shown to all students. We should add a checkbox when creating a student document type that can be checked to require it for all students.

When this is unchecked, it will only show to students that are part of a criteria that is requiring that student document (i.e. specific WBL Types or Career Planning Activities)
