Add an option to track what type of check in you are doing for an On-Site Visit

In most states, they do not do regular physical on-site visits. The majority of these will be a phone call, but email, or virtual meeting should be options. We should add a drop-down for Meeting Type with the following options:

Phone Call

In-Person Meeting

Virtual Meeting

Email

Other

We may also want to reconsider the name of it.
