Add additional prefill options to Forms

EDIT: If we have a field, it should be a prefill option.

For Students:

• Address (we would need to add the ability to store this info)

• Date of Birth (we would need to add the ability to store this info)

• Age (calculate from DOB)

• Middle Initial (calculated from Middle Name)

For All:

• School Phone Number

• School Fax Number

• School Website

For WBL:

• Job Title
