Add a way to have optional forms for WBL like an incident form

This would probably need to be a new form type such as As Needed. Maybe this could just be shown as a tab in the form submissions modal window.

This would also need to either be added to the WBL Type settings or another setting added to manage all As Needed WBL forms.
