Add a field to Programs to identify its PSN (Program Serial Number)

This is used in states like Michigan for state reporting. The state assigns PSNs once a new program application is approved. PSNs are unique codes representing a specific CIP code located in a specific building.

This should also be added as a form filed prefill option.
